- a permanent USA resident card ( green Card or valid US Visa )
- flight reservations
- hotel reservation or invitation letter
- letter of employment and bank statement
- letter from medical insurer stating coverage outside of the US for
up to $37,500 or "unlimited". Be advised that the amount MUST be
